Create a question with type Table (Exclusive) .
In the Table (Exclusive) question, first, add question type Row. This row will be the heading of the table. First row always be for header. How to add Table header row?
Then add checkboxes question type to add row with checkbox choice.

Note:

Table (Exclusive) functionality only work when all child questions marked as voluntary as marked in screenshot with red border.

Checkboxes >Row > Choice questions
