Add Public/Private Disclosure for each indicator (Versions: 4 / 4.5)

Rationale:
Before this release the disclosures were only possible to be added at a page level, however there was a request from one of our clients to have disclosures at indicator levels so that the company users can decide the disclosure for each indicators.

On click of Public / Private button on each of the indicators the selection can be modified. Administrators can also decide to configure an indicator as "Mandatory Public" which will not allow the users to change the disclosures. This can be done at the time of setting the questionnaire in the template.

Response Logs (Versions: 3 / 4 / 4.5)

Rationale:
This feature is developed so that the Administrators of Probench can view the history of all the responses. This helps them to investigate on who modified the responses and when. This also provide the complete trace of user behavior while giving the responses.

Locking Template (Versions: 3 / 4 / 4.5)

Rationale:
This feature was developed to prevent any accidental changes that can be made to the template during the live cycle.

So once the template is locked the users would not able to change anything on the template and system would throw an error message saying "Template is locked".

Also if the user unlocks the template than its logged in the acitivity log that X user has unlocked the template in case its needed to audit about any of the changes.
